    Mucormycosis (zygomycosis) of renal allograft

    Fungal infection is relatively common among renal transplant recipients from developing countries. Mucormycosis, also known as zygomycosis, is one of the most serious fungal infections in these patients. The most common of presentation is rhino-cerebral. Isolated involvement of a renal allograft is very rare. A thorough search of literature and our medical records yielded a total of 24 cases with mucormycosis of the transplanted kidney. There was an association with cytomegalovirus (CMV) infection and anti-rejection treatment in these patients and most of these transplants were performed in the developing countries from unrelated donors. The outcome was very poor with an early mortality in 13 (54.5%) patients. Renal allograft mucormycosis is a relatively rare and potentially fatal complication following renal transplantation. Early diagnosis, graft nephrectomy and appropriate antifungal therapy may result in an improved prognosis for these patients

    Smart Hospital Management System: An integration of enterprise level solutions utilising open group architecture framework (TOGAF)

    A significant portion of the Hospital Information Systems currently consists of various individual legacy applications that have to be integrated, to deliver a more unified solution. The performance, reliability and other factors of these applications can alter the performance, reliability and other characteristics of integrated Solution, the Smart Hospital Management System (SHS). The actual evaluation of these parameters of these applications is outside the scope of this document. The SHS being an infrastructure component relies heavily on the actual resources made available to it for its proper functioning, operation and maintenance. This article aims to deliver an approach in architecting solutions which can be utilised as framework to address common issues in integration of enterprise level solutions. The methodologies discussed in TOGAF version 9 are utilised to demonstrate the feasibility of proposed solution. This paper introduces the problem space/scenarios, constraints, requirements, enablers, risks, sample legacy application architectures and proposed integration solution presented with TOGAF components. The growing number of waiting lists, rising pressure on medical professionals and accountability for medical negligence are only part of the motivation to take initiative towards holds a core model integration strategy in various legacy infrastructure systems. © 2010 IEEE

    A new series of non peptide angiotensin (A-II) receptor antagonist has been prepared. This N-(biphenyl methyl) imidazoles e.g. Some new 4'-{5-amino-2-[2-substituted-phenylamino)-phenyl-methyl]-benzimidazol-1-ylmethyl}-biphenyl-2-carboxylic acid derivatives were synthesized by 2-( α -hydroxy benzyl) benzimidazole was converted to 2-(α-bromo benzyl) benzimidazole by reacting with HBr and Anhydrous ZnCl2 Schiff bases react with biphenyl carboxylic acid with different substituents amino group cyclocondensation with appropriate reagents. Differ from the previously reported and related compounds in that they produce a potent hypertensive effect. The compounds synthesized were identified by 1H NMR, 13C NMR, FAB Mass and FT-IR spectroscopic techniques.     In this study we have synthesized some Benzimidazole derivatives (2-{6-Chloro-5-nitro-1-[2-(1H-tetrazol-5-yl) biphenyl-4-ylmethyl] 1H-benzoimidazol-2-yl}-phenyl)-(Substituted-benzylidene)-amine and screened for their antihypertensive activity. 4-chloro-o-Phenylenediamine was condensed with anthranilic acid in presence of Polyphosphoric acid and different aryl aldehydes compounds with biphenyl tetrazole ring. The presence of specific functional group were analysed by IR spectroscopy, The determination of structure for the synthesized compounds by NMR and Mass spectroscopy 13C NMR, 1H NMR, FAB Mass.     Role of maternal serum ferritin as a predictive marker in intrauterine growth restriction

    Background: Intrauterine growth restriction (IUGR) is most common and distressing complication for both obstetrician and neonatologist. Measurement of maternal serum ferritin has also been used as a predictive marker of increase risk of IUGR. In pregnancy, ferritin level decreases with advancing gestation. Its lowest level is seen around 30-32 weeks of gestational age after which its concentration reaches plateau level.Methods: Total 326 antenatal women enrolled in the study. Maternal serum samples of all women were taken at 25th week and again at 30-32 weeks in trace free mineral evacuated tubes for assessment of serum ferritin by chemiluminescence. Mean of both values was calculated.Results: Mean ferritin value of women with average for gestational age neonates was 15.49 ng/ml and women with growth restricted neonates was 19.71 ng/ml. The women with mean serum ferritin above 20 ng/ml, were 6.26 times more likely to have asymmetrically growth restricted baby and 4.47 times more likely to have a symmetrically growth restricted baby when compared to women with serum ferritin value less then <20 ng/ml.Conclusions: In our study negative correlation was found between the value of serum ferritin and neonatal birth weight. In future large randomized control trial is needed to found association between maternal serum ferritin and IUGR.

    Simultaneous Segmentation and Filtering Via Reduced Graph Cuts

    12 pagesInternational audienceRecently, optimization with graph cuts became very attractive but generally remains limited to small-scale problems due to the large memory requirement of graphs, even when restricted to binary variables. Unlike previous heuristics which generally fail to fully capture details, another band-based method was proposed for reducing these graphs in image segmentation. This method provides small graphs while preserving thin structures but do not offer low memory usage when the amount of regularization is large. This is typically the case when images are corrupted by an impulsive noise. In this paper, we overcome this situation by embedding a new parameter in this method to both further reducing graphs and filtering the segmentation. This parameter avoids any post-processing steps, appears to be generally less sensitive to noise variations and offers a good robustness against noise. We also provide an empirical way to automatically tune this parameter and illustrate its behavior for segmenting grayscale and color images
